  • Completion of detailed falls assessment and management plan in line with NICE guidanceDelivering rehabilitation to patients to help them recover from their hospital admission and return to their home addressWorking in partnership with OT’s and support workers to deliver functional rehab in order to promote independence and facilitate timely discharge planningCompletion of clinical triage of referrals to determine if patients are suitable for admissionCompleting moving and handling assessments of patient on admission, post fall or due to a change in conditionPrescribing and completing exercise programmes with patientsDelegating interventions to support and junior staffCompleting home visits when required and discharge home visits for all patientsProviding supervision and peer to peer support to junior and support staffMFT is England’s largest NHS Trust with a turnover of £2.5bn & is on a different scale than most other NHS Trusts.
