There is an exciting opportunity available for a Patient Flow Coordinator to join our Palliative care team.

We are interested in recruiting a friendly and caring individual who can think quickly on their feet and enjoys being busy everyday to join our growing team.

To provide administrative and clerical support to the Palliative care team , taking the responsibility for efficient flow of patients journeys with the ultimate aim of effective and timely patient care.

1.Information Responsibilities
  • Responsible for the accurate and timely entry of Pallative care patient data onto the palliative care shared drive. Identifying issues and ensuring that they are resolved by liaising with clinical and management teams, both within the palliative care Service and across the wider
Trust.
  • Ensure standards for clinical data and quality metrics within the service are met.
  • Compilation and dissemination of information to support clinical teams in managing performance and adapting clinical practices.
  • Provide data to other information users at all levels throughout the Trust and to appropriate external users as instructed.
  • Carry out ad-hoc analysis of data as requested by staff throughout the Trust ensuring understanding of data provided and advising of its appropriate use as needed.
  • To ensure that the GDPR/ Data Protection Act recommendations relating to the provision of information are always followed.
2. Data Collection and Processing
  • Assist the Trust in its auditing information systems for the accurate recording of all patient activity.
  • Update the Trust Information system to correct known data errors or complete missing information on patient records to include demographics and activity data.
  • Obtain information from external and internal sources to improve the completeness of the Trust’s data.
  • Validate and assess the standard of data making corrections, advising of findings, and carrying out further investigations of issues, working with appropriate Trust Managers.
  • Assist with data requests to support service improvement, audit, and research within the Palliative care service
3) Other Information Duties
  • Help and support to other staff within the department as required.
  • Ensure all processes have written current procedures and that they are followed and always maintained.
  • Cover the work of colleagues during periods of absence or peak workloads.
