Driving licence and access to own vehicle is required due to the essential travel required

You will be working as part of a team to provide support to our homeless clients. You will love helping others and enjoy supporting individuals to achieve their goals and to become more independent. Day In The Life Of An Outreach Worker

Provide intensive, personalised support for a small group of complex high impact users (HIU’s) with very high numbers of multiple hospital emergency department attendances, in order to reduce their multiple contacts with the health and adult social care sector

Work with complex mental health, alcohol and drug users who frequently present at the emergency department

Encourage and support people to access primary healthcare appointments within the community and other appropriate agencies

Engage with known former rough sleepers, who continue to be at risk of homelessness due to addiction issues

Provide an initial assessment to identify client needs and produce person centred outcome and support plans

To provide personalised support that is flexible, responsive and supports the client both practically and emotionally. This may include support in managing and reducing alcohol and/or substance misuse, managing health conditions, improving mental health and wellbeing, homelessness, developing social networks and relationships, or development of practical self-care and living skills

Work flexibly with partners across the health and social care sector to the overall benefit of the client

Clear verbal and written English

Good IT and keyboard skills

Polite, assertive manner, ability to self -motivate

Passion for working in a client/customer facing role

Knowledge of the benefits system and tenancy legislation

Have an interest and genuine concern for homelessness and related issues

Knowledge of voluntary and statutory agencies
