Care is delivered to our patients (clients) in their own homes by carefully selected and trained care teams who are supported and managed by this role which sits within the experienced Clinical Team. We care for clients with a range of conditions such as high-level Spinal injuries, complex respiratory conditions, Strokes, Neurological conditions, end of life and other complex conditions.

Overview of the role

You will form an integral part of our nursing team delivering high quality complex and specialist care management to a caseload of clients. You will oversee the care and therapies and deliver training and supervision. You will work closely with the carer team to support the delivery of high-quality homecare. You’ll be a positive role model for staff offering support and guidance whilst maintaining best practice and clinical standards. You will report to the Director of Nursing and Quality and work within an experienced and dedicated nursing team. The individual will demonstrate a flexible attitude and willingness to embrace change as the specific duties of the role may develop and change in line with business needs.

Main duties

  • To manage the care for group of patients (clients) and their carer teams.
  • To plan and deliver training to staff in practice and assessing competency utilising a structured approach.
  • To clinically plan and supervise the delivery of high-quality care for your caseload of clients in their homes.
  • To provide mentorship, supervision, educational and practice support to your care staff teams in their places of work.
  • To actively participate in the direct care of your clients in their homes, providing safe, effective, high quality and compassionate care.
  • You will be required to provide care and support to any of our clients as required
  • You will provide emergency cover, to support and guide carers, troubleshoot/resolve complex or difficult scenarios and any other supportive duties required in the interests of the service.

Care, therapy and management

  • To actively case manage clients and their care and staff teams to monitor and address all issues or changes to ensure holistic and client-centred care.
  • To act as a role model in all clinical practice, demonstrating compassionate and dignified care and upholding the principles of patient safety and privacy and dignity.
  • To promote positive risk management and use clinical experience and care judgement.
  • Work flexibly within the client’s home environment and their wishes.
  • To recognise any changes in the patient’s condition, acting appropriately using clinical judgement.
  • To ensure care plans, activities, and risk assessment are accurate and reflective of needs for the client caseload.

Training and Development

  • Responsible for care staff training and assessment.
  • Organising and tracking clinical skill and client-specific training and following up where required.
  • Deliver training to staff in practice as well as in the office and assessing competency utilising a structured approach and documenting.
  • Accountable for own professional practice and responsible for acquiring, developing and maintaining competencies and skills.
  • Recognise and address own limitations and learning by organising own training, accessing available courses and learning from other professionals.
  • Work collaboratively with staff and support them through the assessment process.
  • Interprets best practice from experience, colleagues and networking and applies to own work to promote safe, caring and effective care.
  • To support staff and provide a safe environment for clinical supervision, reflection and spot checks to aid learning and development.
  • To address poor practice sensitively by following up with training and action plans.

Information Technology & Administration

  • Learn to use various innovative company care, communications and technology systems
  • Process accurate information, document and update relevant care systems
  • To manage own diary around role commitments, responsible for ensuring quality targets
  • Actively engage with the development and use of company systems to aid the provision of high-quality care for clients and to create a supportive and responsive environment for all staff.

Communication

  • Communicate, deliver and present training plans to staff at all levels
  • To be approachable and facilitate trusting and supportive relationships with staff
  • Forge strong partnerships with staff and form strong teams
  • Impart company caring values and objectives
  • To adapt communications to suit the learner, client and need
