Paragon Home Healthcare is a specialist Home Care provider and are seeking a Care Coordinator to join our Richmond office team.

Job Overview of the Care Coordinator

As Care Coordinator, you will form an integral part of our Richmond Branch team delivering high quality complex and specialist care to clients. As Care Coordinator you will be responsible for planning and coordinating staff rotas and carer teams. You will liaise with staff and clients to forward plan rotas and ensure clients receive safe and effective care. You will report to Service Manager and strive to maintain the highly rated and award-winning service the company provides. You will work closely with the Recruitment & HR, Clinical Teams and directors as needed to support the delivery of outstanding-quality homecare to clients.

Responsibilities

Build and develop strong professional relationships with clients and staff

Provide exceptional customer service to clients and staff

Represent the company’s service in a professional manner in all communication

Communicate effectively with teams, clients, care staff and families to plan rotas

Support and retain care staff, acknowledging the vital role they perform

Coordination

Arrange advanced rotas for staff and clients using the company rota planning system

Send out rotas to care staff and clients weekly and as changes occur

Support staff on how to use company apps and systems

Working closely with the Clinical team to organise staff training and support

Monitoring and reporting on staff attendance using Electronic Call Monitoring (ECM)

Working closely with other teams to plan and coordinate staff

Addressing coordination and other relevant requests

Vacancies and Recruitment

Build carer teams for clients ensuring safe and reliable care

Planning staffing capacity to fulfil new clients on service

Working with the recruitment consultant to focus recruitment and prioritise vacancies

Produce carer profiles, organise and support meet and greets with clients

Compliance and Payroll

Complete payroll checks

Support staff on used of apps and systems

Keeping finance updated with leavers

Others

Take part in office on-call rota outside normal office hours

Take part in duty officer rota within business hours

Escalating to management as required

Complete documentation, administrative tasks and payroll checks and duties

Maintain accurate records and produce reports as and when requested

Use various internal and external company applications and IT systems

Deputising as required and assisting other teams as required

Work in an organised and efficient manner and maintain high standards

Maintain professional relationships and work as part of a team

Cover care visits if required

Undertake additional duties as needed
