The purpose of the Living Well Service is to provide person-centred support to older people and provide them with a range of information, advice and active support to improve their health and wellbeing to enable them to live independently for as long as possible within their community. This will include people living with dementia, armed forces veterans and those with caring responsibilities.

The Living Well Co-ordinator will be responsible for developing a person-centred approach and initiating and developing support plans for older people across the county. They will work with specialist colleagues, internal and external partners including multi- disciplinary Neighbourhood teams and Primary Care Networks to improve health and wellbeing outcomes for individuals.

  • Experience with older people or other relevant work experience (i.e. health and social care or with vulnerable groups and individuals). Is essential along with a good education and the ability to communicate with all types of people.
  • Being able to use Microsoft office packages is essential, organised with strong administration skills.
  • Able to structure own work and work independently.
  • Can work as part of a team. A flexible and self-motivated approach – must be able to show initiative.
