St Martins is working in partnership with Together and Change Grow Live to provide support for individuals engaged with specialist drug and alcohol service across Norfolk.The aim of the partnership is to provide a county wide team that will work together to develop comprehensive support plans regarding the housing & treatment needs of individuals struggling with substance misuse with a significant impact on their housing needs.These roles will be fully embedded within the Norfolk Integrated Housing and Community Support Services (NIHCSS) across Norfolk.We Are Looking For
  • Someone to deliver outcome and recovery focussed services within a Psychologically Informed Environment (PIE); to enable a meaningful personal and social recovery for those under the service, increase and enhance social inclusion through supporting service users to engage with a wide variety of community organisations and services.
  • Someone to be proactive in preventing crises and reduce the need for urgent and emergency care including acute inpatient admissions and minimise lengths of stay in hospital.
  • Someone to support engagement in substance misuse programmes, promote treatment adherence and aim to reduce co-morbid drug use.
  • Evidence of emotional resilience and tenacity to push ahead to find the appropriate solutions, even when faced with resistance.
  • Experience with supporting people who have poor mental health, addictions or who are ex-offenders.
