Job OverviewWe have a permanent vacancy for a qualified Health Visitor working 30 hours within the North Pembrokeshire Generic Health Visiting Team. The post holder will work in the St David’s area.The post holder will be responsible for managing a defined caseload of patients, the assessment of health needs and the development, implementation and evaluation of programmes of care to meet the needs of the child and their family following the Healthy Child Wales Programme.The post holder will need to be committed to developing a high degree of standards of care and actively seek and promote professional working relationships with other statutory and voluntary practitioners. The post holder will be passionate about promoting specialist knowledge and advice to families enabling them to lead healthy lifestyles, prevent illness and to give their children the best possible start in life.A positive attitude to team working is essential.Main duties of the jobIn order to apply you will need to possess a Health Visiting qualification and be registered in the appropriate part of the NMC register. Our 11,000 members of staff provide primary, community, in-hospital, mental health and learning disabilities services for around 384,000 people across a quarter of the landmass of Wales.
