In Carmarthenshire, we are seeking an experienced, enthusiastic & delivery focussed project manager to help us further our ambition of Integrated and work with us on an ambitious programme of urgent care & system flow improvement.

This role will work across both Health & Social Care, you will be able to work with a variety of stakeholders and partners to translate vision and strategy into enhanced and integrated front line delivery which makes a meaningful difference to the Health and Wellbeing outcomes of our population.

This role will be critical to the delivery of a fast acting community team that will impact on urgent care and Health and Social Care system flow.

We are looking for someone who can balance clear project processes and reporting with the development of positive working relationships across the system. You will demonstrate a track record of professional delivery, manage complex change projects and be able to evidence the impact of change delivered.
