Humber Teaching NHS Foundation Trust’s Award winning Health Trainer service are looking for an enthusiastic, dedicated individual who is passionate about helping / supporting people to become healthier and change their lifestyle within the East Riding of Yorkshire as a Health Trainer. The post advertised will be to working with our team in the Bridlington and Driffield area.

Health Trainers give one-to-one and group support, help people to overcome barriers to improve their lifestyle and provide on-going encouragement and motivation.

Experience in working in health promotion is desirable and an understanding of a variety of areas including but not exclusive smoking, healthy eating, weight loss, physical activity, sexual health, oral health, emotional wellbeing and substance misuse.

The post holder must be flexible to work daytime and evenings, including weekends. You must also be required to commute across the geographical boundaries of East Yorkshire.

To improve the health in the local community through:
1. Identifying and engaging with clients from agreed target groups
2. Supporting clients to identify their health needs and what barriers prevent them from making healthy choices.
3. Encouraging clients to increase their ability to make better health choices and supporting them in initiating and sustaining appropriate behavioural changes using a mutually agreed Personal Health Plan.
4. Helping clients to identify, find and use appropriate (health) services
5. Recording and reporting activity and results
6. Notifying manager regarding clients who may require specialist support around more complex or contentious issues.

We employ approximately 2,800 staff across more than 79 sites at locations throughout Hull, the East Riding of Yorkshire, Whitby and Scarborough. We deliver Mental Health Services, Specialist Services for Children, Forensic Services, Community and Learning Disability Services.
