Job summary
Working under the supervision of the Clinical Care Coordinators (CCCO), you will support the everyday Practice operations. The CCCO Team plays an important role in working with the elderly and learning disability residents in the local care homes, including those with long term conditions. You will support in providing routine care for these residents. The CCCO team acts as a key point of contact between care homes and Practices. Building relationships with professionals, clients, residents and family members is essential.
The CCCO Team supports the Clinical Directors and member practices in coordinating key activities including access to service, advice, information and ensuring health and care planning is timely, efficient, and patient centered as well as supporting digital initiates.
Please note that you will work across all four of the Practices within the PCN.

- Work alongside the well developed Care Coordinator Team to provide support for care homes in the PCN

- NVQ level 3 in Health and Social Care
