We are looking for two Early Intervention Health Visitors to join our diverse team of health visitors who make a difference every day to the lives of families in Lambeth and Southwark. This is a great opportunity for experienced health visitors who are interested in furthering their skills in to Early Intervention specialty.

Working within either Lambeth or Southwark, you will be based in our main office, within community clinics and agile working. You will work within the Health Visiting Service, working closely with Community Staff Nurses, Child Development Support Workers and other skill mix staff. Together, you will be providing nursing support and care to local families.

The post holder will provide packages of care/interventions to families with
multiple/complex health and social needs in their homes as well as at local facilities, utilising a case management approach and structure parenting interventions to improve outcomes for the families.

The post holder will deliver care and early intervention strategies to support families under the supervision and support of the Senior Early Intervention Health Visitors and Health Visiting Matron.

  • To provide specialist, proactive and autonomous clinical case management
including assessments and care coordination and case management of families with complex health and social needs, using evidence based knowledge of early intervention, parenting, mental health and prevention strategies with the support of the Senior Early Intervention Health Visitors and matron.
  • Participate in monitoring the quality of interventions, advice, and follow-up care.
  • To deliver the enhanced Healthy Child Programme and intervention packages to children and families on the vulnerable families caseload
  • To keep up to date with new techniques and developments for the promotion and maintenance of good practice.
  • Utilise tools to improve the quality of the service to achieve agreed standards through regular clinical audit of practice.
  • To work with wider skill mix team members of the vulnerable families service to organise, plan, facilitate and evaluate structured parenting programmes for identified families.
