The Mendip District nurse hub has a vacancy and we are looking for a dynamic Band 5 to join the team.We are a busy hub who’s main focus is keeping people at home and you will ultimately be responsible for triaging phone and email referrals received from acute hospitals, GP practices and other disciplines for patients in need of community nursing care within the Mendip neighbourhood. The District nurse Hub currently consists of the hub nurse and a team of admin support who are managed by a team leader and operates 8am - 6pm Monday to Friday with occasional out of hours cover.In addition to professional referrals you can expect to answer calls from families, carers and the patient themselves so will require a caring and empathetic manner.Assisting the teams with unplanned clinical visits when required.The DN hub nurse role triage’s all new referrals sent, which at times need further exploration but once accepted a clear clinical decision should be made identifying the most appropriate plan for that patient.The hub receives requests from various sources including community hospitals, the acute trusts, GPs, hospice care, rehab services, rapid response, carers and family members.As a registered nurse you must have an empathetic manner and excellent communication and organisation skills, with an ability to build strong relationships with other services. You must be flexible and responsive to managing the changing needs of the district nurse service while possessing an ability to make difficult decisions when required.Somerset NHS Foundation Trust is the first NHS trust on the England mainland to provide community, mental health and acute hospital services.
