We have an exciting opportunity for an experienced community nurse to join the Place Based Team in Rugby.

The post holder will be required to manage a busy workload and staff within the routine work flow, delivering high quality, holistic nursing care to patients in their own home.

We are looking for a competent and organised clinician with at least 2 years community nursing experience and skills in assessing patients, managing the complex care of long term conditions, prevention of hospital admission where appropriate, and supporting end of life patients.

The successful candidates will be motivated and enthusiastic, and must be able to demonstrate strong leadership ability and an innovative and flexible approach to service delivery.

The successful candidate will be responsible for leading a team of clinicians and a variety of patients supported by the wider team.

You will hold mentoring qualifications and have evidence of continual professional development including management of patients with long term conditions.

In return the applicants will be supported by a knowledgeable and experienced team with regular clinical supervision together with any relevant training.

Minimum Qualification (or working towards) – RGN. Registered with the NMC, Mentor qualification (or equivalent), Health Needs Assessment qualification.

Applicant must hold a full driving licence and have access to own vehicle (Car Lease scheme in operation).

The District Nurse is responsible for the assessment and management of the health needs of an identified population and for the development, implementation and evaluation of programmes of care within a range of target groups including end of life care, complex health needs and patients identified as being at risk of hospital admission.

The District Nurse will clinically supervise and provide support to a team off staff – being responsible for the development and competencies of staff within their team. To include performance development reviews, clinical supervision and recruitment and selection.
