Position has become available in the Diabetes service in the Community.

We are looking for an autonomous practitioner Diabetes Nurse Advisor band 7 who has a wealth of knowledge and experience with at least over 24 months within this area. You will work as part of an integrated community team and with acute, primary and social care partners to prevent unplanned hospital admission wherever this is feasible.

The role requires some teaching/education to patients in the community. You will be expected to actively be involved in the multidisciplinary team meeting as part of hospital avoidance

Enfield is a large borough on the northern border of London with easy access to many green spaces including pleasant parks and footpaths through the green belt. Enfield also has easy access to many and varied shopping centres as well as good public transport.
  • To perform a comprehensive Diabetes assessment of adults referred to the Diabetes Nursing service with diverse presentations and complex physical and medical conditions, to provide a clinical diagnosis and develop and deliver an individualized treatment programme
  • To hold responsibility for own caseload and be responsible for working without direct supervision. Supervision takes the form of regular clinical reasoning sessions including peer and case reviews. Access to advice and support from a more senior diabetes nurse is available when required
  • To undertake all aspects of clinical duties as an autonomous practitioner
  • To undertake evidence-based audit and a project to further own and teams’ clinical practice. Make recommendations to clinical lead or manager where appropriate. Contribute to the implementation of specific changes to practice or contribute to service protocols with support and guidance from more senior staff
