ClinicalSupport Worker–Community Response Team (CRT) for bothStratford& Leamington Teams

An exciting opportunity has arisen for an enthusiastic Band 3 Community Support Worker to come and join us in our Community Response Teams. We have two teams situated in Stratford upon Avon and Leamington Spa working across the county supporting patients that have identified health and social needs as a result of a change in their health condition. The service is committed to provide high quality care to provide an immediate response, aiming to prevent avoidable hospital admissions by providing support to patients to remain independent within their own home and facilitating safe discharges home from hospital. The team works together to support patients in the community and work closely with social services, GPs, community nursing teams, patients, relatives and carers.

To be successful in this role you must hold a valid driving licence and have access to your own car.

The Community Response Team operates across the hours of 08:30-21:30, 7 days a week, including weekends and bank holidays. Shifts are allocated pro rata, we offer a mix of early, late and long days.

Clinical Support Workers are members of the Multi disciplinary Team, who undertakes professional duties in the Patient’s homes to ensure provision of high standard care, assisting with all aspects of daily living and promoting independence.

The Band 3 Clinical Support Workers will undertake high quality, caring and compassionate personal care and associated duties under the direction and supervision of the registered staff.

As a Band 3 Clinical Support Worker (CSW) you will make a valuable and important contribution to patients’ quality of life, supporting them to remain safe at home.
