Do you enjoy the challenge of working out in the community and within the comfort of patients own homes?The role will support the Team Leader and the team itself in providing excellent care to patients in our NeighbourhoodOur team is motivated and enthusiastic and works closely with our patients and their carers and families as well as the local surgeries, Secondary care and Adult social care.We pride ourselves on embracing the challenges of the ever changing demands of our population and hard to reach communities.The role will support the Team Leader and the team itself in providing excellent care to patients in our Neighbourhood.You must be a Registered General Nurseand community experience is desirable but not essential.Responsibilities include: End of life care, Venepuncture, medication administration, wound care, leg ulcer management, catheter care, bowel management, continence assessments, diabetes management.Supporting the hospital at home service to keep patients who are acutely unwell at home via close monitoring including administering IV medications, regular blood tests, weights and observations.As an organisation, the NHS offers a wide range of benefits including flexible working, pension contributions, market leading annual leave allowance, career progression and regular conversations, not to mention our Blue Light Card and NHS exclusive discounts.The benefits of working in Somerset include the idyllic countryside, with our areas of outstanding beauty and stunning coastlines.
