Job OverviewHywel Dda University Health Board are currently looking to recruit a full time Community Mental Health Practitioner to join the North Pembrokeshire Community Mental Health Team based at Bro Cerwyn, Haverfordwest.The post is permanent and will be for 37.5 hours per week (part time would be considered).Main duties of the jobWe believe it to be a hard working but friendly, supportive and nurturing environment, with many established and experienced practitioners.This post offers an excellent opportunity to work in an interesting and developing service environment. You will have considerable opportunities for training and professional development.Our services work through an assessment & recovery approach with people accessing secondary mental health services for severe and enduring mental illness. With an exciting opportunity to be involved in an adapting and transforming structure to our mental health services.The post holder will be responsible for the assessment of care needs, the development of programs of care and the implementation and evaluation of these programs of care for a defined clinical caseload including the role of a Care Coordinator within the Care Treatment Planning under the Mental Health (Wales) Measure 2010.The post holder is required to work without direct supervision for their delegated caseload. The post holder will have effective communication skills and develop positive working relationships between all healthcare professionals, statutory and voluntary agencies, maintaining good communication across all boundaries.The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.Working for our organisationHywel Dda University Health Board is the planner and provider of NHS healthcare services for people in Carmarthenshire, Ceredigion, Pembrokeshire and its bordering counties.
