We are looking to recruit an experienced and enthusiastic Healthcare Assistant who will support the Community Nursing in Middlesbrough, Within Greater PCN, but will also be part of the larger Tees teams, within the Community Care Centre.

To provide a high standard of health care to patients within their own homes, as part of the integrated community nursing service the post holder will contribute to the admission avoidance agenda by providing high quality care to patients.

He/she will be responsible for carrying out designated work, much of which will involve direct patient care. This care will be delegated by trained staff and the post-holder will be expected to work under the indirect supervision of trained staff. The post holder will work in a person centred way to achieve clear outcomes and adhere to trust and team objectives.

Successful candidates will be experienced in working in a health care setting however community experience would be an advantage and candidatesmust havea National Vocational Qualification Level III (in Care / Health & Social Care).

The successful candidate should be enthusiastic and be able to work on their own and as part of the team.

It is essential for the role that the candidate has the ability to travel independently.
