Job OverviewThis post is a fantastic new opportunity to take part in a new role developed to improve outcomes for those with co-occurring mental health and substance misuse concerns. The role is to work with individuals that meet the criteria for secondary mental health services and require CTP who also have substance misuse concerns. To provide support and advice to both services in working with substance misuse and mental illness, as well as providing strong communication links and shard joined up approach to working with individuals.The successful applicant will be expected to be able to assess and manage substance misuse clients by applying the concepts of harm minimisation approaches, relapse prevention, motivational Interviewing, detoxification programmes as well as providing mental health care and treatment according to their clinical needs, and in accordance with up-to-date evidence-based practice and relevant Trust clinical protocol and policies.The successful candidate will be a core member multi-disciplinary team, liaising with community mental health services as well as participating in the provision of on-going assessments, screening, care planning, treatment and support of people with drug and alcohol issues, who also experience mental health issues, to promote health, independence and recovery.Main duties of the jobYou will be expected to: -- To hold a limited caseload for those identified as having established Substance misuse issues within the CMHC/CMHT.
- To provide assessment, care planning, review of risks assessments and evaluating care of patients that fits the criteria for co-occurring substance misuse and mental health needs
- Deliver appropriate interventions to meet the needs of patients in our care, and connect individuals to appropriate community resources to support recovery and independence.
- To support the MDT in delivering safe, effective treatment for patients with substance misuse and mental health needs.
- To be part of a culture that focuses on improving outcomes across the care pathway for patients, and joint working with CMHT and CDAT colleagues.
- To keep updated with national policy and legislation, attending relevant health board meetings.
- Manage workload effectively and responsibility for planning and delivering of individuals care and discharge process.
- To provide compassionate service and excellent patient experience to all in out population, working to the “no wrong front door” ideology that we will support any person to get the help they need.
