Job OverviewSomerset NHS Foundation Trust is looking to recruit an enthusiastic, organised and proactive pharmacist to join the Medicines Management Team as a Clinical Pharmacist, working across mainly community hospital services.A sound knowledge of prescribing and medicines management issues is essential with excellent decision-making, time management and problem solving skills. Ideally you will have significant post registration pharmacy experience, in a range of different care settings and have completed, or be on your way towards finishing your clinical certificate. We encourage and support staff to pursue further qualifications.Somerset NHS Foundation Trust provides services across the county so the base will be confirmed upon appointment. It is essential to be a car driver with access to own transport.Hours: up to 37.5 hours per week - part-time and flexible working applications will be considered.Main duties of the job
  • A mainly ward based role in Community Hospitals inpatient wards throughout Somerset, undertaking clinical review of medication activities and helping the ward teams.
  • Contributes to the development of the medicines optimisation service to make the best use of the staff and assist in the co-ordination of admission and discharge processes.
  • Providing advice and guidance to Community Health Services for patients, carers and other healthcare professionals within and outside Somerset NHS Foundation Trust.
  • Assist Senior Pharmacist Leads in the clinical supervision of Community Rehabilitation Service (CRS) and Neighbourhoods Directorate Pharmacy Technicians.
  • Assist in the delivery of training to Foundation Year Trainee Pharmacists, Pharmacy Technicians and other professionals in matters relating to medication.
  • Participate in and provide patient/carer medication education sessions.
