Join our self-managing team in Chatham. This is a great opportunity for those who feel they no longer have the time to give basic holistic care to their patients.

The teams will have professional autonomy and responsibility, working to support and empower each other to deliver the highest standard of person-centred care.

Community nursing in Medway is part of exciting changes as we adopt the principles of working within a Buurtzorg model of care. We are also looking for experienced staff with effective communication and organisational skills to manage both staff and patient caseloads. We are offering one band 6 role as well as two band 5 roles.

To work as a member of a self-directed neighbourhood nursing team that delivers person centred integrated care in the community. The aim of the service is to support people to remain as independent at home as long as possible, proactively facilitating self-care.

To work as an integral part of a team of 8 -12 nurses providing consistent care to a defined community caseload of patients 365 days a year, 24/7 hours a day. This requires flexibility and commitment to maintain effective service provision for the team’s caseload.

The role requires close collaboration with the GP and other health, social care and voluntary organisations including but not limited to therapists, mental health, social workers, care navigators, case managers etc.

  • You must be a 1st or 2nd level registered nurse
  • Our neighbourhood teams are based within the local community throughout Medway enabling them to get to know the local community whilst also working very closely with local GP surgeries and other services
