Exciting opportunities have arisen at Southampton City Councils CQC Outstanding rated Urgent Response Social Care Service

Unqualified Care Manager 37hrs Monday - Friday

(£30,451 annual salary,£40 per month contractual car user and 45p per mile)

Urgent Response is an Integrated Health and Social care Service offering short term Rehab & Reablement support to adults in their own homes across the City of Southampton. Assisting individuals to maximise their independence with daily living tasks.

Job involves

Assessing for on-going care needs

Funding requests to meet assessed needs

Care planning

Triaging safeguarding concerns

Liaising with other professionals involved with the individual

Referals / signposting to outside agencies

Must hold a clean UK driving licence and have access to a car.

  • This post requires an Enhanced DBS check.
