This is an exciting opportunity to join the Primary Care Team in Hywel Dda University Health Board to work as part of the GMS sustainability team to support and develop the four Health Board Managed Practices as well as working to support Practices with sustainability issues to optimise the delivery of services to meet the needs of individual patients and national and local priorities.The successful candidate will be motivated and driven with a passion for developing General Medical Services. You will have excellent communication skills and a track record for working successfully with GP practices to identify proactive and positive solutions to sustainability pressures.The Successful Candidate Will Need ToEnsure the delivery of efficient and effective general medical services in Hywel Dda managed practices as well as providing advice and support to practices where sustainability has been identified as a significant concern.Will lead the local development and implementation of the Primary Care Model for Wales, considering utilising and developing new and emerging roles and systems to provide services for urgent and routine care within general practiceOperationally lead and manage the teams within the Managed Practices whilst working closely with the Heads of GMS and the Assistant Director of Primary Care to lead on innovation and effective delivery of the Primary Care core functions across Hywel Dda Managed Practices.The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.We Do This In Partnership With Our Three Local Authorities And Public, Private And Third Sector Colleagues, Including Our Volunteers, ThroughHywel Dda University Health Board is the planner and provider of NHS healthcare services for people in Carmarthenshire, Ceredigion, Pembrokeshire and its bordering counties.
