Somerset Intermediate Care Service- bedded pathway - Assistant Practitioners- Band 4

Tadham Unit- based within Glastonbury Care Home

Are you an experience Assistant Practitioner who has a passion for carrying out rehabilitation programs with people to enable them to reach their desired outcome

We are looking to recruit an experienced Assistant Practitioner who is looking for a new challenge and would like to join our new service with the care home environment focusing on longer /slower stream rehab to enable the individual to reach desired outcomes

The successful applicants will be part of a Multidisciplinary team, who are highly passionate and innovative about rehabilitation

The successful applicants will work alongside Therapist’s and members of the MDT to support the daily work within the care home environment. As part of the team you will be working with patients who require therapy intervention to remain as independent as possible within the care home environment in order to return home to own environment .

Main aims of the service is to
  • provide patients the opportunities to return current home environment following a episode which has brought them into a bedded Reablement/ rehab units.
To support transition from the service another key aspect of the role will be to have understanding of other community services within the county and build/form relationships with Community teams/Voluntary Sector and Adult Social Care team’s around the county to allow a seamless transition.

Hours of work for this post is between 8am and 6 pm, 7 days a week. You may be expected to work in shifts during the week alongside to ensure the full day is supported from a therapy point of view. weekend working will be required

Their will be a expectation to work along side the Therapy team to provide training regarding enablement/rehabilitation approach within the home .

day to day Line management will be provided by Band 6 therapist’s who are working on the unit
