An exciting opportunity has arisen to join the newly formed Dementia Wellbeing Community Team in Hywel Dda University Health Board (HDdUHB). This team is part of a developing Dementia Wellbeing Pathway service, which is a wrap-around support for people living with dementia & their families to ensure there are increasing levels of expertise available following on from receiving a diagnosis and through the journey of dementia to maintain and enhance people’s quality of life.

This is an unique opportunity for post-holders to work with a range of Multi-Disciplinary professionals (inc. psychologist, speech and language therapist, dietician, advanced pharmacist, advanced occupational therapist and advanced nurse practitioners), to shape innovative service provision.

The post-holder will be embedded within the respective multidisciplinary team, with the intention to work alongside the team to flexibly share their expertise and extend their influence of both into people’s own homes (inc care homes) and within health, housing and social care services, at primary, secondary and tertiary care levels. In addition the expectation is that the post holder will support the professionals in delivering person-centred (needs led) care and to champion the equality and rights for this client group and their carers.

Applications are invited from individuals experienced in dementia, who are interested in working as part of multidisciplinary team as well as an Older People’s Mental Health Psychology service, which actively supports professional development and is committed to excellent patient care and service improvement. We have strong links to the South Wales Clinical Psychology Doctorate training programme.

The ability to drive is essential as the post holder will be expected to travel across the Health Board to visit patients at home and between sites. Mileage allowance is payable and pool cars and a lease car scheme is available.
