Job Overview

The post holder is identified as a clinical expert and is required to have a high clinical profile ensuring the provision of effective and efficient care. At this level the post holder is expected to work as a highly skilled senior practitioner and to have expert knowledge.

The primary focus of the role is to work as an advanced, autonomous practitioner providing patient-centred clinical care, encompassing the skills of assessment, examination, diagnosis and treatment of patients. The Advanced Practitioner will work across the 4 pillars of practice – Leadership & Management, Clinical, Education, Research and Development, as outlined in the Framework for Advanced Nursing, Midwifery and Allied Health Professional Practice in Wales 2010 (FANMAHPPW).

The post is intended to provide improved outcomes for patients by improving timeliness and consistency of care.

Main duties of the job

An exciting opportunity has arisen for an Advanced Nurse Practitioner in one of the practices managed by Hywel Dda University Health Board which is Ashgrove Medical Centre located in Llanelli in Carmarthenshire.

This progressive and challenging role offers the post holder the opportunity to provide high quality, patient-centred care in a general practice setting.

The post supports and promotes the role of the Senior Practitioner through innovation, clinical skills and diagnosis, treatment, review and referral.

Llanelli is a very diverse set of communities in urban, semi-rural and rural settings. The Cluster Network in Llanelli serves a population of approximately 66,755, which is the second largest in Hywel Dda University Health Board which is served by a mix of urban and rural practices.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

Hywel Dda University Health Board is the planner and provider of NHS healthcare services for people in Carmarthenshire, Ceredigion, Pembrokeshire and its bordering counties. Our 11,000 members of staff provide primary, community, in-hospital, mental health and learning disabilities services for around 384,000 people across a quarter of the landmass of Wales. We do this in partnership with our three local authorities and public, private and third sector colleagues, including our volunteers, through:

Four main hospitals: Bronglais General in Aberystwyth, Glangwili General in Carmarthen, Prince Philip in Llanelli and Withybush General in Haverfordwest;

Seven community hospitals: Amman Valley and Llandovery in Carmarthenshire; Tregaron, Aberaeron and Cardigan in Ceredigion; and Tenby and South Pembrokeshire Hospital Health and Social Care Resource Centre in Pembrokeshire;

48 general practices (four of which are managed practices), 47 dental practices (including three orthodontic), 99 community pharmacies, 44 general ophthalmic practices (43 providing Eye Health Examination Wales and 34 low vision services) and 17 domiciliary only providers and 11 health centres;

Numerous locations providing mental health and learning disabilities services;

Highly specialised and tertiary services commissioned by the Welsh Health Specialised Services Committee, a joint committee representing seven health boards across Wales.
