Social workers registered with SWE.

Experience of using Liquid Logic, working with people with mental disorders, arranging hospital discharges, collaborating in the arrangement of s117 aftercare services, conducting MCAs and best interest decisions. Required to support the community mental health teams in Liverpool, working with an MDT to plan safe and effective discharges for people leaving hospital. You must have experience of using Liquid Logic.

Experience of carrying out Care Act assessments, mental capacity assessments, and taking best interests decisions, co-producing Care Act care and support plans, and participating in the planning for, and commissioning or, s117 aftercare services. You will work as part of a team of social workers and social care assessors, you will receive 4-6 weekly supervision and have access to relevant training.
