Day to day responsibilities include:
  • Manage caseload of high support needs service users
  • Conduct risk assessments and provide trauma-informed support
  • Collaborate with external agencies and attend multi-agency meetings
  • Respond to crisis situations and write incident reports
  • Ensure service users have access to maximum benefits
  • Report to Operations Manager
  • Document all interactions and maintain accurate client files.
  • Participate in ongoing training and education to improve skills.
  • Attend team meetings and case conferences to review veterans’ progress.
  • Carry out administrative tasks like paperwork, data entry, and scheduling.
  • Assist with admissions and move-outs.
