Job Purpose

To provide high quality, personalised support to D both at home and in the local and wider community.

You will be working within an established team that provides support for D.

We are looking for people who have experience working directly with complex health requirements and who feel comfortable using a range of different equipment. Preferably respiratory health issues.

You must undergo an Enhanced DBS check for this role

Driver Preferable: (Due to insurance would need to be someone 25 years of age or over)

£9.50 per hour

There is a three month probationary period to check suitability of the role.

Accountable to: The Support Provider

Responsibilities

Provide emotional and practical support.

Create opportunities for social and leisure activities. Support D to make and develop new friendship as well and maintain old ones.

Help meet personal care needs sensitively and appropriately to a high standard, including personal hygiene, assistance with laundry, and preparing meals and drinks.

Supporting D to take any prescribed medicine, when required.

Enable D to achieve independence as far as possible in all areas of their life by providing appropriate information, opportunities, training and support.

Assist D in devising, implementing and reviewing her support plans.

Participate in rotas, which may include Bank Holiday working.

Assist in risk assessments, follow health and safety procedures.

To attend all training in order to support D with all her personal care needs.

Ongoing Support.

As D’s health is constantly changing it is all team members’ responsibility to adapt with the changes in order to support D to the best of their ability and to prevent D being at risk.

Ensure that D is supported as far as possible to exercise her rights in the following areas: Privacy, choice, participation in decision making affecting her life, expression of her own cultural identity.

Liaise with other professionals on behalf of the individuals we support.Maintain contact with a wide range of professionals, e.g. General practitioners, dentists, and therapists etc.

Liaise with other service providers, colleges and other places of interest.

Keep records and participate in administrative tasks.

Participate in the opportunities provided for training and development.

Participate in regular supervision to: receive support, increase knowledge, identify training needs, evaluate work performance, aid and assess professional development.

Attend staff meetings and training courses,

Work within, and promote, the policies and procedures.

Help cover the work of D’s team, during absence, vacancies.

Be supportive, respectful and empathic to team members and all others involved in providing D’s support.

Basic Requirements

A staff member may have some of the following skills on appointment but would be expected to attain them all within 12 months.
  • A good understanding of how to support someone in a person centred way and taking their lead.
  • Ability to provide personal support in manner that is respective and maintains dignity.
  • Ability to support people in their chosen activities.
  • Communication skills – verbal and non-verbal and an ability to build a relationship that is meaningful and empowering.
  • Ability to support people in moving & handling safely in relation to yourself and others.
  • An understanding of person centred support.
  • Basic numerical and literacy skills.
Higher Requirements

In order to achieve (higher status) you would be expected to be using a combination of at least three of the following on a frequent basis in your place of work
  • Awareness of ensuring that D is supported well in all expects of her life and supporting her to maintain and build new relationships.
  • Recognized skills in supporting to maintain a healthy life which includes her approach to meals and her dietary preferences.
  • Awareness of complex health support issues and delivering the support required, including keeping active and healthy whilst also knowing when to rest and recoup.
  • Experience in nursing care and support due to D’s respiratory conditions.
