Job Overview

We have an exciting opportunity to join our Community Stroke Service Team as a Band 7 Team Lead/Advanced Practitioner AHP/Nurse, to work across the Scarborough and Ryedale locality.

You will be expected to lead and support the provision of a seamless Stroke service from the local acute hospitals into the community. The team provides assessment and rehabilitation within the community setting for adults, from 18 years upwards who have had a Stroke. We work closely with professionals, services and agencies across the whole Stroke pathway.

You would be part of a small, dedicated, knowledgeable multi-disciplinary stroke team. You will be joining the team at an exciting time of service development towards implementing the National Service Model for an Integrated Community Stroke Service, with a focus on creating a robust early supported stroke discharge service.

Main duties of the job

You will be responsible with the Service Manager and Clinical Lead/Matron for the provision and development of the Community Stroke Service across Scarborough and Ryedale. You will work within a multidisciplinary team, including Speech and Language Therapists, Occupational Therapists, Physiotherapists and support staff and have line management and supervisory responsibilities for junior staff within the stroke team.

You will hold a caseload of patients and be accountable in your own area of clinical responsibility, ensuring the delivery of evidence based clinical interventions and practice

Applicants should have a strong value base and promote holistic person-centred care. Previous experience of working within Stroke services and community experience would be advantageous. Additional specific training can be provided.

The post holder needs to be flexible, enthusiastic, have good time management skills, confident IT skills, and work well in a team as well as independently. A car driver and access to own vehicle use with business use insurance is essential as this is a community based role.

Qualifications And Knowledge

Essential criteria

  • Professional knowledge acquired through degree and Post Graduate specialist qualifications
  • Be able to demonstrate equivalent knowledge or experiential learning/experience in service development, managing change, leading and managing staff, plus specialist training and short courses to Diploma level relevant to area of practice: e.g. dysphagia assessment and management
  • Current HCPC professional registration
  • Degree in Occupational Therapy, Physiotherapy, Speech and Language Therapy or Nursing
  • Educated to Masters Level or Masters level/ Post Graduate modules supplemented by specialist accredited training and experience in specialist

Desirable criteria

  • Advanced/expert clinical qualification
  • Accredited leadership/management qualification
  • Specialist/advanced understanding/application of relevant managerial and leadership aspects of the service area

Experience

Essential criteria

  • Evidence of post-registration professional practice
  • Proven leadership/management experience, able to demonstrate examples of positive impact/change within service delivery/practice at an advanced level
  • Evidence of continuing professional development (CPD) relevant to the service area
  • Evidence of specialist/advanced practice skills and able to demonstrate the impact of this on practice change/development

Desirable criteria

  • A breadth of professional practice including specialising in the field where the post is held
  • Ability to work across team/organisational boundaries developing and maintaining multi-professional and multi-agency partnerships

Skills And Competencies

Essential criteria

  • Excellent and effective inter-personal skills and experience in supervising/managing staff
  • Work within the culture of improving working lives and working time directive
  • Ability to effectively commute between all various Trust sites

Desirable criteria

  • Able to demonstrate effective communication skills at all levels i.e. strategically and locally
