FULL TIME STAFF NURSE 37.5HRS Mon-Fri BAND 5/6 EQV Due to a pending retirement, we are looking for an experienced, enthusiastic staff nurse to join our nursing team in delivering high quality nursing care to our practice population. We are a friendly and approachable team and are proud to be rated as outstanding by CQC. We are based in the centre of Whitley Bay and our clinical system is TPP Systm1. We have a team of 7 GP Partners, 4 Practice nurses and healthcare assistant. Our admin team comprises of 20 members of staff. We have a list size of 12700 and offer a personalised list system to ensure excellence in care and continuity. As well as general practice nursing duties (including, but not exclusive to, women’s health, screening, vaccinations, injections and health promotion), this post has a substantial focus on delivering effective individualized care to patients with long term conditions. Ideally we would like someone with expertise in chronic disease management and in particular experience in managing asthma, COPD, CVD, diabetes heart failure and hypertension however we would support a nurse willing to develop the role and undertake further training to full fill these duties. You will be aware of and focused on meeting QOF targets whilst confidently delivering patient centred reviews and supporting self care and management. Experience in filing and actioning test results would be advantageous but not essential. You will be supported by our senior Practice Sister and the GP Partners, to grow and develop the role and your skills. NMC registration required and a full D&B check will be required.
