ART+ is an innovative and tem providing therapy and care to patients from Bath and North East Somerset who are discharged from the RUH. It utilises a discharge to assess model and is a reiteration of the previous project known as ART (Active Recovery Team).
ART+ work with three local domiciliary care providers to provide reablement and care at home. AHP staff set goals, review patients and hold a daily MDT via MS Teams. The team is led by a B7 Therapist.
