Job Overview

This is an excellent opportunity to join our team as a Virtual Ward Specialist Physiotherapist

We are looking for an experienced physiotherapist, to work with our Virtual Ward team providing clinical input in order to facilitate to remain safely at home. The role includes integrated MDT working targeted at people living with frailty, multi-morbidity and/or complex needs to help them stay independent and healthy for as long as possible.

To succeed in this role you will need to be passionate about community healthcare provision,able to tackle key priorities with enthusiasm & have the tenacity to deliver in an ever changing environment. As a senior therapist you will have management and supervision responsibilities for junior staff & students.

Part time / flexible working options will be considered for this post and can be discussed if required.

Main duties of the job

The main duties of this role include -

  • To provide high quality and timely clinical triage for referrals coming into the service, providing advice and guidance to referrers and liaising with internal teams and partner organisations to support patient care delivery
  • To be an integral part of an MDT consisting of Nurses, Physiotherapists, Occupational therapists and Support workers delivering patient centred care to maximise independence, confidence and ability.
  • To demonstrate specialist clinical skills in practice for patients in need Care and support
  • Support patients with individualised goal setting, education and care plans to work towards fulfilling their potential in a targeted and realistic way via delivery of evidence based clinical interventions and practice in partnership with patients, relatives, staff, managers, and externally with other agencies.
  • To provide support/advice regarding service development and the delivery of care, in partnership with service users, carers, staff, managers and other agencies.
  • To provide line management, supervision, support, guidance and training to peers, and students

Person specification

Experience

Essential criteria

  • Evidence of sound post-registration professional practice
  • Experience of complex case load management, within specialist area.
  • Evidence of continuing professional development (CPD) relevant to the clinical area at a specialist level of care

Desirable criteria

  • Experience of working in Community based physiotherapy
  • Experience of using SystmOne

Skills And Competencies

Essential criteria

  • Effective inter-personal skills and experience in supervising and mentoring
  • Ability to demonstrate ethical values and attitudes within a culture of equality and diversity
  • Can commute effectively to facilitate community based visits including transportation of equipment required for clinical interventions.

Desirable criteria

  • Ability to carry out basic observations and calculate NEWS2 score
  • Ability to identify and set up home based assistive equipment according to individual patient need

Qualifications And Knowledge

Essential criteria

  • Degree/ Diploma in Physiotherapy
  • Current HPC professional registration
  • Post graduate diploma qualification in relevant specialist area or equivalent knowledge and experience
  • Specialist understanding of relevant clinical practice, standards and protocols within identified clinical area

Desirable criteria

  • Member of specialist interest group/network, locally or regionally
