The Role

Targeted Review Team are keen to recruit enthusiastic, motivated, experienced social workers to join a newly forming Targeted Review Team; the team is being set up with the sole purpose of carrying out targeted reviews across the Adult Social Care Service from 18 onwards. It will be a fast paced, busy team with pre-determined aims, targeted pieces of work.

Our aim in the Targeted Review Team will be to:

Review support plans to ensure have the right level of support.
Apply a Strengths Based Approach, professional skills and knowledge to work with people with lived experience and their families to develop a support plan which best meets need and desired outcomes.
Maximise the person with lived experience independence skills, guide them through to the right level and type of support.

About you:


  • We are looking for someone who is experienced in building and sustaining relationships with people with lived experience and their families.

  • Must be confident and able to make decisions independently.

  • Be able to work at a fast pace as well as ensuring Assessments continue to be completed to an excellent standard.

  • Have strong values towards person centred practice, positive risk taking, cultivating self- actualisation, self-determination.

  • Be an excellent communicator

  • Understand the key principles and apply those strengths based principles/approaches when completing assessments, identifying needs, risks and outcomes with the individual.

  • Have an excellent working knowledge of applicable legislation including being able to appropriately apply legislation is essential.

  • Be able to work flexibly and effectively in partnership with a range of voluntary and statutory agencies as well as having good time management and organisational skills.

  • Be knowledgeable and have the confidence to work across the full spectrum of groups of adults including older People, Learning Disability and Mental Health.

The team will be made up of a Team Manager and four Social Workers it will be a compact team undertaking a particular scope of work but it is hoped will become a bonded team who work closely together, supporting each other to work towards the team goals.
