Position Title: Social Worker (Hospital Discharge to Assess)
Salary: Grade 7 £30,451 to £33,782 (Bar at £32,910) per annum (plus 6.7% flexibility payment)
Contract Type: Permanent full time
Hours: 35 per week - 7 hour shifts between 8.00 am to 8.00 pm Monday to Sunday
Work Location: Citywide - North and South Manchester

The hospital discharge to assess team support citizens through the hospital discharge processes. Our input, if required, starts in hospital and continues into the community where a full assessment will take place in an appropriate setting.

You must be willing to be flexible and able to work at a fast pace. You will need to be a confident practitioner with a good understanding of the Social work role to ensure effective multi disciplinary working.

We work flexibly throughout the whole service, this includes; D2A, Crisis response, intermediate care and the hospital. Social workers will work within the multi disciplinary team both in and out of the hospitals to ensure a holistic approach to assessment.

This post is a designated Essential Car User. Therefore, you must hold a full valid driving licence at all times and provide your own car for use at work. You will receive the Essential Car User Allowance.

The post is covered by the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (amended in 2013). Appointable candidates will be required to apply for a Disclosure and Barring Service (DBS) check.
