Cheerful and committedSenior Social Workers to work within the ICAT (Intermediate Care Allocation Team) and Hospital Discharge Teamin Adult Social Care at Lancashire County Council.

Our friendly and dedicated Adult Social Care Service are keen to welcome three new Senior Social Workers to the ICAT and Hospital Discharge teams. We are seeking knowledgeable individuals with the correct qualifications and appropriate experience to support and enhance the service we provide. The focus is primarily getting the best possible outcomes for our service users to ensure they are safe and supported upon their departure of a hospital setting. Your compassionate and empathetic manner will allow patients to feel empowered, by building trust and developing positive working relationships. In addition to the contribution you’ll make supporting those leaving hospital, you will also be managing a small number of less experienced colleagues who will welcome your guidance and mentoring.

What We Are Looking For…

Passion – you will be passionate about people with your caring attitude and want the best possible outcomes for them - this role is very much person centred so you’ll need to be approachable and committed to providing excellent care – the aim is hospital avoidance, essentially putting measures in place to prevent a visit to the hospital - this will be achieved by working alongside community teams and putting in provision for crisis care and other appropriate services; assistance such as this will be massively beneficial to the lives of many vulnerable adults – you will be someone who can be depended upon so able to deliver a great service in a timely manner and to the highest standards

Empathy & Resilience – all of the patients, carers and agencies that you will encounter need to be communicated with effectively, eliminating any delay in response – co-ordinating delivery support with each service requires excellent organisation skills so you will need to be resilient in terms of chasing up any outstanding actions – it can be a daunting experience to be in hospital or requiring additional care, therefore providing an individualistic service for each case will allow patients to achieve independence whilst meeting their specific needs and personal choices – we want an influential character to carefully assess the behaviour of others, becoming an advocate for patients and carers alike – your empathic and caring nature will facilitate a positive experience for all concerned, ensuring freedom for service users is well within reach

There is an expectation of you to work 1 in 4 weekends, up to 18 per year with varying shift patterns which can be discussed – this role is 37 hours per week working 5 out of 7 days.
