Detailed job description and main responsibilities

Clinical Responsibilities:

  • To be accountable for the delivery of the Social Work service on the NRU ensuring the delivery of an effective and co-ordinated admission to discharge service for patients.
  • To ensure that the agreed quality standards of service, based on professional guidelines and competencies are maintained by the SW and the Social Work Assistant.
  • To be professionally and legally accountable and responsible for all aspects of own professional practice.
  • To provide a comprehensive assessment of individual patients in need and their carers, including those in highly complex situations, in conjunction with the MDT.
  • To intervene and provide services to achieve change, through provision or purchase of appropriate levels of support and care.
  • To take the lead role in managing complex patient discharges, co-ordinating the MDT and external organizations to ensure that the appropriate accommodation, care and financial resources have been arranged.
  • To arrange and chair case review and/or discharge planning meetings with patients, their support network, relevant MDT members and outside agencies as appropriate.

  • CQSW, Degree in Social Work

Experience

  • Minimum of 4 years working post qualification experience
  • Sound working knowledge of care management and Care Act 2014
  • Experience of working with patients with complex needs
Desirable criteria
  • Neurorehabilitation experience in a hospital or community setting
  • Experience of working with a diversity of client groups
