An *EXCITING* opportunity has arisen for an experienced clinician to provide the role of Primary Care Mental Health Practitioner as a Band 6. This is an opportunity for a qualified mental health nurse to become part of the Primary Care Network (PCN) and work across 3 of Mid Essex’s PCN’s supporting GP’s with Dementia diagnosis.

Essex Partnership University Foundation Trust (EPUT) and Mid Essex Clinical Commissioning Groups are working in collaboration to create an Enhanced Primary Care Team as part of a new approach to the delivery of Primary Care Services.

We are looking for a successful applicant who is:

  • An experienced registered mental health nurse at band 6
  • Preferred to have the Nurse Identification Qualification (or able to work towards this qualification)
  • A competent, confident and proficient practitioner
  • Who is organised and able to work creatively and effectively in a busy and demanding role
  • Have the ability to travel across the locality for service delivery

Our Community team work with individuals with the dementia and frailty and are responsible for assisting with the delivery of patient centred care under the supervision of a senior member of the team. This will include providing practical support to service users and their carers, guidance and education and assisting in their management and care based upon the individual needs.

You will be expected to work collaboratively with key partner organisations, service users, carers, clinicians and other practitioners within the multi-disciplinary team in delivering services; providing a 7-day a week, 24-hours a day service for 365 days a year, working shifts where appropriate and operationally required.
