Main duties of the job

To play a role in helping to further develop the Rheumatology Service, with particular emphasis on improving and strengthening integrated partnership working with primary care colleagues.

To provide the necessary health education and support to nurses and allied health professionals and general practitioners working in community and primary care settings regarding the monitoring and managing of rheumatology patients with chronic inflammatory joint disease, thereby aiming to achieve a reduction in the number of patients having to attend hospital out-patient clinics.

To develop strong links with primary care colleagues, participate in the development and transfer of routine care for rheumatology patients from secondary to primary care, thereby enabling high risk patients to be seen and managed by specialist consultants.

This new post will also have continuing overall responsibility for a defined caseload of rheumatology patients through nurse led clinics providing high standards of care with each patient.

To enable the rheumatology specialist team to identify areas of audit and research that is required and in collaboration undertake R&D developments as part of role.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
