It’s an exciting and challenging opportunity, we’re looking for an enthusiastic, self-motivated and highly organised individual with exceptional strategic and operational management skills. You will have a broad and flexible range of skills including excellent communication skills, including report writing, ability to work under pressure, organised, full knowledge in using Microsoft software and have a methodical approach when dealing with external care referrals.

The Assessment and Referral Manager will have extensive experience in the Health and Social care sector, ideally having worked with people with learning disabilities and/or mental health needs at management or above level for 1 year or more.

The applicant will need to have experience in assessing the needs of people with disabilities and be able to make recommendations on support needs, identify risk and how to manage this and be able to identify compatibility with the services in our portfolio. The Applicant will be required to:

- Monitor & respond to referrals from various external sites

- Keep our tracking system updated

- Book and take the lead on assessments,

- Complete and submit the needs assessment, with support package.

- Build relationships with commissioners, Stakeholders and attendance at provider forums
