REABLEMENT WORKER

Location: Glastonbury, Street, and surrounding areas.

Salary: £10.42 - £10.85 per hour, plus paid travel time and mileage. £300 welcome bonus!

Due to our continued success, we are looking to expand our Reablement team to provide a first-rate Discharge to Assess (D2A) service within the Mendip District, in particular Glastonbury, Street, Frome, Shepton Mallett and surrounding areas. We are looking for motivated, enthusiastic and resourceful individuals to join our amazing Somerset team to provide a high-quality service to the people who need it the most.

Working in partnership with Hospitals across Somerset, D2A is a service provided on a short-term basis, where individuals who are well enough not to need an acute hospital bed, but may still require care services, are provided with funded support to be discharged to their own home, with a support plan tailored to the client’s needs and outcomes.

Your new role as a skilled Reablement Support Worker will see you providing high-quality person-centred care delivered competently and safely in our customer’s homes where you will support them in their day to day life and achieve the goals set for them to be as independent as possible.

What does your new role involve?
  • Assisting with personal care, including washing, dressing and toileting where required, all the while encouraging independence.
  • Motivating clients and building their confidence to achieve the goals they have set.
  • Identifying risks and completing risk assessments to keep everyone safe on an ongoing basis.
  • Supporting clients with their mobility and working with a multidisciplinary care team to achieve goals in care plans.
  • Maintaining accurate work records and assisting with the development of individual support plans.
  • Developing effective working relationships with other health and social care professionals.

  • Knowledgeable care professionals who have experience of caring for vulnerable persons with a wide range of complex needs.
  • Sensitive and considerate individuals with a natural desire for improving the quality of lives for others, by providing a high quality service to people who need it the most.
  • Proactive and self-motivated people with great initiative who have a clear ability to identify issues and implement solutions to a wide range of challenges facing our clients when they return to their own home.
  • Patient and empathetic individuals with a good sense of humour, who are able to remain calm and focused - even when under pressure.
  • Respectful and friendly individuals who work just as well on their own, and as part of a team.
  • Individuals with excellent communication skills who can influence, encourage and coach their clients, tactfully but not forcefully.
  • A full driving licence and/or access to your own transport is essential for this role as you will be required to travel to support our customers within their own homes.
What We Can Offer You In Return

Hours

We have full and part time positions available. Our flexible shifts are between 07:00 - 22:00 every day, with alternate weekend working and will include some split shifts.

Pay

We offer a competitive wage starting at £10.42 per hour and rising up to £10.85, depending on experience, paid travel time at £8.92 per hour, paid mileage of 40p per mile, a £300 welcome bonus after successfully passing your probation period.

Holiday

Annual leave amounts to 28 days per year, including Bank Holidays, pro-rata for part-time hours.

Uniform, Training and PPE

We supply your uniform and all PPE, as well as an extensive and on going training programme.
