An exciting opportunity has arisen for an innovative programme manager to facilitate the delivery of the Personalised Care and Support Programme across all directorates in Sheffield Teaching Hospitals involved in the care and treatment pathway for patients with cancer. They will ensure patients from diagnosis to surgical, non-surgical and follow up care have access to the Macmillan Living With and Beyond Cancer (LWABC) model of care. The model’s interrelated elements include:-

Risk stratified pathways to reduce the current model of hospital based follow up, where appropriate, and enable person centred aftercare and support, closer to home.

Personalised care interventions, including holistic needs assessments (eHNA), treatment summaries, health and well-being events and cancer care reviews.

Self- management support options to enable people living with and beyond cancer to live as full a life as possible, including healthy lifestyles, return to work, financial and emotional support

The role will work with multi-disciplinary service delivery teams across STH to:

Implement holistic needs assessments at key points in the cancer pathway for patients, utilising the electronic holistic needs assessment (eHNA) across all tumour sites and directorates in STH who have a pathway for cancer diagnosis and treatment.

Support the implementation of risk stratified pathways (consultant follow up, remote monitoring and self-management) across all relevant tumour sites and directorates in STH.

Increase and improve the utilisation of treatment summaries across all relevant tumour sites within STH.

Enable people affected by cancer to receive personalised care and support, with a particular focus on disadvantaged groups to address inequalities in health.
