We are looking for an experienced, motivated and dynamic Physical Activity Trainer to join our enthusiastic and progressive team in delivering health services for people experiencing chronic pain. The community based Pain Management Team provides an extensive service across Hull and East Riding of Yorkshire.

CHCP Pain Management Service is committed to providing the highest quality of holistic care working in partnership with patients, their care givers and other health care providers. You will work well as an integrated member of a multi-disciplinary team supporting service delivery of individualized treatment plans for patients in a variety of health care settings.
