The co-ordination of the patient journey is a critical factor in ensuring the timely and effectivedelivery of care. This role provides a means of improving quality of that care for individual patients by ensuring the prospective tracking of their journey, timely involvement of the specialist Multidisciplinary Team (MDT), early identification of bottlenecks with proactive management of actions to addressWe are looking for a highly motivated individual with good organisation skills to join the Cancer Patient Pathway Co-ordinator team.The post holder will be required to track the progress of cancer patients through the cancer journey, completing electronic records to assist you. You will be expected to proactively manage all aspects of the patients pathway, intervening & escalating where necessary, to ensure timely appointments, diagnostic investigations and transfer to tertiary centres as appropriate are achieved, in compliance with the national Cancer Waiting Times targets. The post holder will also provide clerical and administrative support to the specialist Multidisciplinary Team (MDT) meetings, preparing & recording data and care outcomes on clinical systems.You must have considerable experience of working in an NHS environment including the use of clinical systems i.e. EPR/ICE/iCRIS. You should also have excellent word processing/typing skills as this post requires a significant amount of clinical data entry & ‘live’ typing in MDT meetings.The role also requires the collection & recording of specific clinical data for all the National Cancer Audits.You will be required to have excellent communication skills as this post requires liaison across the many multidisciplinary services and the ability to work independently.An ability work to a high degree of accuracy and attention to detail is essential.We understand that whether you’re just starting out or have plenty of experience, moving jobs can be both exciting and daunting, but you’ve made a great choice in considering joining Calderdale & Huddersfield NHS Foundation Trust (CHFT).
