Salary: Agenda for Change Band 3 £27,948 - £29,468 pa including HCAs
Hours: Full time - Approximately 150 hours over four weeks. Worked in 12 hours shifts – including Weekends and public Holidays.
Length of Tenure: Permanent

It is an exciting time at Trinity as we launch our new community services, which make up our new clinical hub. The new services have been developed to streamline the coordination of care for patients, address the evolving palliative and end of life care needs of our local population and maximise the flexibility and responsiveness we can offer.
We are looking for an experienced, enthusiastic, and highly motivated individual to take up the post of a palliative Care assistant within Rapid Response Team.

You will be a key member of the Clinical Hub Services, and the wider multidisciplinary team working across Royal Trinity Hospice’s community catchment area supporting patients with urgent and complex specialist palliative care needs wherever they call home.

Key Responsibilities

  • To support the functions of the Clinical Hub including:
. Specialist Community Nursing

  • To provide personal care within any existing care plans, and in accordance with the Clinical Nurse Specialist, and medical care prescribed by the GP.
  • To provide personal care and support to the families and carers of individual patients in the place they call ‘home’ across the Trinity catchment area, being sensitive to their changing physical, psychosocial and spiritual needs.
  • To communicate and liaise with the patient’s usual care team including District Nurses, other palliative care support such as the 111 Roaming Service as well as the Clinical Hub Team Lead, reporting on the condition and needs of the patients and providing and maintaining clinical records, as necessary.
Skills and experience required
  • National Care Certificate (NVQ in Health & Social Care Level 3 or equivalent) with experience and knowledge of care giving.
  • Able to recognises changes in a patient’s condition and confident in delivering care in a variety of settings.
  • Able to communicate with sincerity and empathy.
  • Enthusiasm and a flexible attitude to working.
  • Ability to work autonomously and as part of a team.
  • Leadership and Management Experience
  • The successful candidate must be able to travel with ease across our catchment area (a current UK driving licence is desirable).
