Main duties of the job

LOOKED AFTER CHILDREN’S NURSE - Edge of Care 37.5 hrs

The post holder will work as the named Looked After Children’s(LAC) Nurse for the Counties of Anglesey and Gwynedd in North Wales. Based within a community health setting and working closely with the Local Authority. The post holder will have professional responsibility to coordinate timely completion and implementation of high quality statutory health assessments and care plans for Looked After Children. The aim and purpose of the post is to provide services in partnership with multi disciplinary teams (School Nursing, Health Visiting, Community Paediatricians and other LAC Nurses) within and external to Betsi Cadwaladr University Health Board, to ensure the best possible health and social outcomes for the child/young person in the care of the Local Authority. The post holder will be an advocate for LAC Children and Care Leavers.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
