LONG TERM CONDITIONS CARE CO-ORDINATOR

Gladstone House Surgery is a busy, friendly, caring practice who are part of Erewash Health Partnership, which includes 9 other surgeries within the Ilkeston and Long Eaton areas, with a list size of 5,000 patients. We have an active Patient Participation Group and are rated Good by CQC.

We are currently looking to recruit a part-time 19hrs per weekLong Term Conditions Care Co-Ordinator.

Summary

Are you good at problem solving and can work methodically?

Have a keen eye for detail?

Have experience of working with the general public?

This role is to provide coordination of health reviews for our patients with long term health conditions and you will work with both our admin and clinical teams in a fast paced environment.

Excellent communication skills both written and verbal are essential and being able to prioritise your workload.

Duties of the Job

Provide coordination and navigation for people and carers across health care servi8ces, working closely with primary care professionals to ensure patients receive a joined-up service and appropriate support.

Assist people to join self-management courses or other help that supports them in their health and wellbeing.

Comprehensively monitor information onto clinical systems (SystmOne), build and ren reports into the clinical system to identify different cohorts of patients with long term conditions and use the recall system
