Young People Link Workers

An exciting position has become available for a full-time Young People Link Worker based in Wolverhampton. The successful candidate will work 37 hours per week and will earn £26,270 per annum.

Liaising closely with GP practices in Wolverhampton, you will be working alongside young people aged 13-17, supporting them to tackle loneliness, isolation, anxiety and depression by connecting them into a broad range of services & interventions to improve their health and well-being.

Role Requirements

  • Youth Work, Social Work, Health or equivalent qualification at Level 3
  • Ongoing commitment to learning in relevant areas such as safeguarding and related issues which may impact on young people.
  • Experience of working with young people
  • Experience of working in a social prescribing model or similar
  • Experienced and motivated to work independently as well as in a team
  • Experience of working in a community setting
  • Experience of managing administrative tasks to include monitoring & evaluation
  • Experience and/or knowledge of local statutory and voluntary agencies
  • Able to offer some flexibility in working hours to fit availability of young people
  • A sound understanding of issues and procedures relating to Safeguarding Children and Safeguarding Adults at risk of abuse
  • A sound understanding of Child Developmental processes
  • A knowledge of & ability to support appropriate pathways relating to emotional wellbeing, mental health and other health needs, social care and educational needs for vulnerable children and young people
  • A proven knowledge of strategies pertinent to young people
  • Demonstrate a working understanding of the principle of equal opportunities as it applies to the context of the post
  • Computer literate & able to use standard software e.g. Word, Excel & Outlook to facilitate working practices, as well as experience of using case management software
  • To be able to work effectively and balance caseload work
  • Excellent written and verbal communication skills
  • Flexible and creative interpersonal approach
  • Experience of working alongside volunteers
  • Able to travel regularly across the City to a range of venues

Role Responsibilities

  • To act as a main point of contact for young people accessing the service.
  • Liaise with appropriate statutory and voluntary sector agencies working in Education (including SEND), Public & Mental Health, Primary and Secondary care, Social Care and Youth Offending Teams and build up appropriate networks that promote timely access to support for young people.
  • To work systemically, e.g. liaise with and include parent/carers, educators, health professionals etc. when appropriate and as agreed by the service users.
  • To build strong working relationships with internal and external services and to promote the link worker/social prescribing model.
  • To attend multi-disciplinary team meetings (MDTs) as required
  • To attend mandatory training and abide by policies and procedures on safeguarding, health and safety and equality and diversity
  • To attend appropriate continuing professional development and training events, team meeting and supervision sessions. (These may be run by any of the partner organisations as deemed appropriate by the service)
  • To ensure young people are made aware of key processes e.g. complaints, information sharing, getting involved, and to make sure that they are asked about how they would prefer information; to ensure special information requirements (e.g. large print, language, braille etc) are clearly recorded on their files and that their needs are met
  • To carry out, from time to time, any other appropriate duties in line with capabilities as directed by the line manager.

This post will require an enhanced Criminal Records Check and Barred List check for the children’s workforce from the Disclosure and Barring Service.
