Overview

Are you interested in supporting children and young people to gain new experiences within their community and develop their independence?

This is an exciting opportunity to lead on developing pathways to adulthood, researching and exploring choice within the local market. Creating links and engaging with organisations and activity providers in the community. You will be involved in planning support for new referrals and ensuring that each person has a person-centred transition plan to take them into adulthood. We support up to 25 individuals between the ages of 14 and 25.

About The Role

You’ll promote skills, knowledge and wellbeing by providing excellent support to young people that enables them to achieve individual outcomes and have a varied selection of interests and relationships. This will include things like building social skills, enjoying clubs and other activities. You’ll also develop a programme of activities build around the persons preferences and needs, you will coordinate activities ensuring the people we support are working towards achieving their outcomes. Some of the individuals communicate through their behaviours.

You’ll provide leadership and day-to-day support to your team of support workers through coaching, performance management.

With a dynamic flexible approach, As some of your work will be directly supporting individuals the role includes some weekend and evening shifts.

About You

You’ll ideally have experience of working with young people. You can you cope under pressure and support young people who communicate through their behaviours. You’ll have level 3 Maths and level 1 English qualifications (or able to demonstrate this capacity through assessment) and a Diploma in Health and Social Care is desirable.

You’re considerate, committed and respect people’s differences. Empathetic, you adapt your approach to individuals’ needs, have good communication skills and a positive, patient approach. You come up with creative solutions to overcome challenges and have a ‘can-do’ attitude.

It’s essential that you have a full driving licence and be willing to use your own vehicle on business – we’ll pay mileage expenses for this.
